I have an old British paper licence, will that be accepted? Do you still have to take a test or does the British licence mean you automatically get issued with a Thai licence?

Yes, it will be accepted and you won't have to do the practical or written test.
You will only have to take braking reaction, distance orientation and colour blindness tests.

That gives me a problem as I am colour blind. I suppose it's back to the International licenc every year.

No problem - it's just dots on a card with numbers.
Take a Thai person with you to stand behind you, then mumble your answer is an incomprehensible brogue, which your Thai friend then miraculously translates into the correct answer for the "examining" official.
I'm not joking; it actually happened when I was there. Everyone had a good laugh about it and the candidate passed!!!

So the year hasn't expired yet - you should be ok in that case.
You need your passport with valid visa and proof of residence, e.g. yellow book, work permit or confirmation letter from immigration.
Plus your old licence(s) - separate for car and motorbike - and a health certificate, obtainable from most doctors' surgeries.
There's a copy shop at the driving licence office for the copies of these documents that you will need.
Photos aren't necessary - they're done there and integrated into the new licence.
Cost for 5-year licence around 500 baht each.

If it expired july 2009 it is 11 months out of date tihs year according to my calender is 2010

You apparently didn't read back far enough and misinterpreted what I said, which was

Where exactly do I need to go to get the license? Want to get this done so I don't have to drive the other way every time I see a cop

Go to Pranburi, the test/license centre is behind the Police station.
I've lost my mind and I am making no effort to find it.

My 1 year Thai temp licence is due for renewal. Do we really need to go through the whole pullava again with doctor's certificate, letter from immigration.. etc?

You need your passport, your old licence, a doctor's cert (30 baht in Pranburi) and proof of where you live (yellow book, work permit or letter from immigration).
