)Office were very friendly, set up an appointment, and told me to bring:
1) Cash or money order--US$175
2) Marriage Cert for Wife or Birth certificate for baby AND COPY
3) Passport AND COPY of info page
4) 1 photo (1inchx1inch)
5) Filled out visa form
6) Copy of ID card or Passport from baby or wife (Depending on what document used in 2 above.
The process was friendly and efficient--we were out of there within 15 minutes.
