My understanding is that it relates to high level corruption that is way beyond the reach of the vast majority.
E.g The award of an airport contract with tax payers money to a company that belongs to a friend of the minister even when the building standard is lower and the overall cost much greater to the tax payer. KhunT & Swampi being obvious examples. At least an airport got built.
In some African counties the national annual GDP is awarded to a company that suspiciously goes bankrupt the next day or disappears off the face of the earth.
I don't know how taking little bribes for minor traffic infringements could be measured.
I doubt very much if border runs and people exploiting loopholes in the European laws will come into it either.
Virtually all business in Hong Kong of old and Singapore is done in what I would call a closed market. As those two countries are so high I guess that is not counted either.
