For multiple re-entry you need form TM8, another copy of your passport and 4800 Baht fee. Photos can be made outside the office, but you can bring also your own photos in passport format.

I think you'll find the fee for multi re-entry is 3,800 THB. Single is 1,000 THB.

To OP: I forgot about photos. Take along some passport sized ones.
