Medical Certificate for O-A Visa ???

Hello folks,
Anyone know where I can download the medical certificate needed to submit for a Non-Immigrant O-A Visa (long stay)?

I checked the Immigration Web-site but it is not listed. All the other required forms are listed.

Due to some family issues I have to remain in the US beyond my Thai Immigration "permitted to stay date", so I need to apply for a new Non-Immigrant O-A (long stay) visa while here.

Some years ago when I initially got my first Non-Immigrant O-A (long stay) Visa, you could download the medical form from the Thai Embassy web-site, but that is down for maintenance now and who knows when it will be up and running.

The Royal Thai Embassy web-site in Washington, D.C. is back up and running ! Sooo I can answer my own post by saying the medical certificate is available there to be downloaded.
