Proving Girlfriend is live in partner

I am in the process of trying to get the live in girfriend onto the company I work for medical BUPA scheme ,but they require me quote to obtain a legal document from the local government authority to prove that she is your common law wife or partner).
We have stayed together for 6 years ,and the company is outside of Thailand.
Has anyone been in the same situation or knows how i can get some paper work to resolve this.

I havn't been in this situation and I'm afraid the only thing that springs to mind is that your g/f gets some police paperwork certifying her residential address. Quite what she'd need to prove this, I don't know.

If you have documents such as bills etc., in both names and then go to your embassy and sign a stat declaration . . .
