Resistant malaria spreads rapidly to Thai-Myanmar border

Post by PeteC »

To be honest, I didn't know the problem in western Cambodia was that bad. Kind of puts a damper on any thoughts about going to Ankor Wat. Also, has it not stayed in heartland Thailand since it crossed 800 KM through Thailand on it's way to the Burma border? Confusing. Perhaps it is indeed here but we have another case of the government hiding the facts. Pete

Deadly malaria that is resistant to drug treatment has spread rapidly from Cambodia to the border between Thailand and Myanmar, raising concerns of an uncontrollable epidemic, scientists said Thursday.

A pair of studies published in The Lancet and the journal Science showed how the disease is moving fast into new territory and identified a region of the parasite's genome that may be responsible for mutating in order to survive.

Malaria is a mosquito-borne disease commonly caused by a parasite, Plasmodium falciparum, that kills up to 1.2 million people a year, according to 2010 estimates by the Institute of Health Metrics and Evaluation at the University of Washington, Seattle.

Malaria that was resistant to treatment with the current standard therapy, artemisinin, was confirmed in Cambodia in 2006 and has since surged 800 kilometers (500 miles) westward to the Thailand-Myanmar border, the researchers said.

By studying 3,202 patients along the northwestern border of Thailand near Myanmar from 2001 and 2010 and measuring the time it took them to clear malaria infections from their blood after treatment, scientists were able to show a steady increase in drug resistance.

The number of slow-clearing infections rose from 0.6 percent of cases treated in 2001 to 20 percent in 2010.

In western Cambodia, 42 percent of malaria cases were resistant between 2007 and 2010, indicating that the Thailand-Myanmar region was swiftly catching up to Cambodia's rates.

"Genetically determined artemisinin resistance in P. falciparum emerged along the Thailand-Myanmar border at least eight years ago and has since increased substantially," said The Lancet study.

"At this rate of increase, resistance will reach rates reported in western Cambodia in two to six years."

The research in the journal Science focused on what was making these parasites different, and found that a region on chromosome 13 of the parasite was strongly associated with slow clearance of infection.

They sequenced the genomes of 91 P. falciparum parasites from Cambodia and western Thailand and compared them to parasites from Laos, where resistance to the latest artemisinin-based drugs has not yet emerged.

They found seven genes that may be responsible for making the parasite resistant to drugs, and which may explain up to 35 percent of the growing resistance in southeast Asia.

"We have now seen the emergence of malaria resistant to our best drugs, and these resistant parasites are not confined to western Cambodia," said leader of the study Francois Nosten, director of the Shoklo Malaria Research Unit that studies and treats malaria in the Thai-Myanmar region.

"This is very worrying indeed and suggests that we are in a race against time to control malaria in these regions before drug resistance worsens and develops and spreads further."

The death toll from malaria has been declining in Africa -- the part of the world worst hit by the disease -- in recent years, largely due to the increased use of artemisinin drugs and the widening distribution of insecticide-treated bed nets.

But if resistance spreads to artemisinin therapies -- used alone or taken in combination with other anti-malarials -- some experts are concerned that a resurgence of drug-resistant malaria could return to Africa.

Researchers say a new anti-malaria drug is not expected to appear on the world market before the next decade is up.

The two studies were funded by the Wellcome Trust and the US National Institutes of Health, and included scientists from Mahidol University, Bangkok; the Centre for Tropical Medicine at Britain's Oxford University; and the Texas Biomedical Research Institute in the United States.

Oh sh*t. Do we know more specifically in which regions around the border?
The Thai-Burma border is pretty long..

This is being widely reported now in the Western media and could easily cause a panic with tourists IMO. The one thing they are NOT reporting is the location along the border, but as HH is about as close as you can get (or at least it is for most tourists) - it is a worrying development. Just as worrying though for Burma which is otherwise set to open up now.

Not wanting to advise against anti-malaria medicine, but I dont usually take any medication/immunisations when going to Thailand...mind you I am only going to Hua Hin/Cha Am and Bangkok and might visit the local lighter jungle areas near the Burmese border. I never thought malaria was a threat in these areas. This article is saying that they're becoming resistant to the drugs...I've never taken them and I've never had malaria...yet.

Mosquitoes are quite smart but they are very poor map readers and have little concept of geographical or political borders.
If indeed the new strain is truly increasing in some areas, it will spread to other areas in SE Asia.
Appropriate clothing, skin creams or sprays and netting are still preffered to pharmaceutical protection.

According to advertising blurb on my Ultra Violet zapper, mosquitoes do not move more than 0.5 Km from place of birth until death.
I have a twin tube zapper outside (the same can be found in butchers and kabab shops in UK) which is great when you hear the crack of another mozzie meeting its end. :guns:
Make sure you site it away from your evening sitting out area, if you buy one.
Might be good idea for next government incentive to place one of these every 0.5 Km.
This would eventually eradicate all mozzies in a green manner without chemicals, save on health bills and is much cheaper than tablet computers.

Be very wary of anti-malerials. My doctor ill advised me to take them first time I came to Thailand, even though I was not going anywhere with a particularly high risk.

I had to stop taking them after my first week in LOS and I still suffer from the side effects which can be very unpleasant indeed. This is 8 years later.

Do you remember what you were taking? I was considering a trip to Angkor Wat a while back and remember reading that Doxycyline was recommended as a prophylactic. Was it that or something else?

I remember there were 2 types on offer. Daily and weekly tablets. The daily ones apparently increased sensitivity to the sun, and as I already burn just by thinking of the sun, I decided on the weekly ones.

Anyway, from what I gather they both can cause pretty horrific side effects (just read the disclaimer in the packet, the list is massive), but I think the ones I took were the worst type.

Sorry I can't be more helpful.

Thanks, Pleng. A little googling should help me know what to avoid - ahhh - maybe Chloroquine or Mefloquine. I know I can take Doxycyline, so that and some good repellent should do the trick.

I've had the opposite problem with a doctor in the UK and had to argue that anti malaria vaccines/tablets are not effective against every type of malaria. As is now being shown with this more resistant strain. The problem with many doctors in the west is that they are paid more - or receive more money - from selling as many shots as possible to patients whether they really need them or not. Vaccines are big business. GP Practices are armed with malaria maps, high risk area info etc, non of which can ever be fully up to date. What they fail to tell patients is to cover up and spray dawn and dusk. There is simply no profit in that.

Yes, very sad state of affairs everywhere in the world now concerning private hospitals where you have to be on your guard rather than just relaxing and trusting the physician will treat you properly....and economically. If my parents were alive they wouldn't believe me if I told them it's now best to research what you think you have, including the treatment and the medications, so in the event you're right you can question a doctor's treatment plan and if it's padded with unneeded procedures/medications.
