Well that seems to be the answer, I have an International Prem account in Sterling, what a bummer, I try to support the country of my birth by having a Sterling account and I end up losing out.
Pretty poor of LTSB offshore that they were unable to answer my questions over the phone and then told me to put my complaint in writing.
At least I know where I stand now or at least I think I do.

Uh generally 0845 numbers generate a small amount of revenue for the companies. Something like 1/2p per minute. Worth having, but certainly not enough to pay the wages, even in out-of-country call centres. If they're using an 0870 or 0871 it might just be enough to pay the wage of an off-shore employee, providing the lines are in use 24-7. Premium rate numbers would of course give them a decent return. But if any bank charges premium rate to call, you clearly go elsewhere!
Anyway, if you're having problems with your bank charging, there are plenty of other ways to transfer your cash with no fee. Many people here seem to be happy with the service that the Post Office is providing. I use a company called caxfonfx.

I don't think that is the answer actually, like you I have an International Premier Account in Sterling and I don't pay for transfers done through phone banking I wonder if it could have anything to do with where your account is, mine is in the IOM, though surely that shouldn't be a factor
