I have a multiple entry "O" visa which expires in October this year. I will be arriving in Thailand in the middle of September. I have 3 questions:
1) Is it possible to extend my Visa for a further year or do I have to get a new one?
2) Can I get it extended (or get a new one) in Thailand or do I have to do this in my own country?
3) Am I allowed to stay in Thailand for 90 days after my arrival date or 30 days after the expiry date of my visa?
I would appreciate advice. Many thanks,

Quick answer to 3)
As long as you re-enter Thailand before your visa expires you will get a permitted to stay stamp for a further 90 days after your arrival date.

You can extend an O visa for a year here like anyone else but you have to qualify and marriage to a Thai or retirement are the usual reasons. So, marital status, age and financial requirements need to be met. If you can't meet them, then no you can't extend for another year .
Extensions are done internally, visas are granted externally.
Assuming you don't qualify for an annual extension here, then your only real option of getting an O visa (whether single or multi entry) will be in your home country. You're not going to get either from a neighbouring country to Thailand as you don't qualify for one.
Thai honourary consulates in your home country are the best places to apply for another multi entry Non O. You don't mention where you're from but that information will help us direct you better.
Your third point has already been dealt with by TimandT. As long as your visa is still valid you'll get 90 days on arrival. After that, it's tourist visas or 30/15 day visa exempt stamps if you want to stay longer.

Arriving mid-September and visa expiring sometime in October means you could get XX extra days over 90 out of it by making a border run a day or two before it expired. Pete

I have a retirement visa. Do I have to provide the same paperwork as before in order to extend it?

I'm confused as you say in your OP that you have a multi-entry Non O. That is not a retirement visa although you may have been granted it because you're over 50. In fact, to be pedantic, there's no such thing as a retirement visa. There is a multi-entry OA visa that is a long-stay visa and is commonly referred to as a retirement visa. Please clarify what type, O or OA, you have. We also don't know what you supplied to get your original visa so we can't tell you if that is sufficient.
In any event, you can extend either visa for a year here in Thailand. If you're over 50, so applying on the basis of retirement, then you'll need to prove either an overseas income of 65K THB a month or savings in a Thai bank of minimum 800K THB or a combination of the two as long as the total is over 800K. If you're going down the savings route, the money must have been in a Thai bank account for three months before application and never have dropped below that amount over those months. If you use the income method, you need a letter from your Embassy in BKK verifying it and quite what they will require is up to them so you need to check. Imm here like you to apply for the extension within the last 30 days of any 90 day/ 1 year permitted to stay stamp.
There are other bits and pieces, but the above are the ones you need to plan ahead for and we can fill you in on the others if you require more help.

My type of visa is Non Immigrant, category "O", number of entries "M". On the visa it says Enter Before 7 Oct. 2011. I will be in Thailand from September to March. After 90 days I assume I will have to do a Visa run. If my visa is not extended then I assume I will only get 30 days stay after the visa run instead of 90 and I would like 90 days in order to avoid frequent visa runs.

Perhaps you would be better applying for a new o visa multi entry before you travel to Thailand - that way you can get the further 90 days you require.
