Extending tourist visa, three entries for 6 months

Can i extend this for a month at a time after the initial 2 months is up at HH immigration, rather then leaving the country every two month

If so how much?

As far as I know... if you have a single entry tourist visa, you can extend it once for 30 days at immigration in HH, then you must leave the country, returning with another tourist visa or on a 15 day (via land border) or 30 day (via airport) entry stamp.
If you have a double or triple entry tourist visa, you can extend as above, but you still have to leave the country before the extension expires, then you return on the second entry and follow the same procedure.

That's right. If you've got a triple entry tourist visa, you'll get 60 days on arrival here and then extend in HH or other Imm office for a further 30 days. Cost 1,900 THB. After that, you must exit and return to get another 60 days (second entry) and get another 30 day extension. That covers you for your 6 months (as good as) and you still have one entry left if you want to use it.
On the note of the 30 day extensions, you don't have to wait until the 60th day to get them. You could go a week early and you won't lose any time. Ie, your 30 day extension will start from when your 60 day permitted to stamp expires, not from the date that you apply for and get the extension.
EDIT Just another point, the triple entry visa is valid for 6 months from date of issue. Make sure any exit/entries you make happen while the visa is still valid. If you made the first two entries work to their full extent of 90 days each (with extensions), your visa will almost certainly have expired so you won't get the third 60 day entry in.

My brother is coming out on a triple entry tourist visa and my question is can he do the exit/re-entry at Ranong and get the full 60 days?

Excuse my ignorance, even though I have lived here more than 25 years, I have not got a clue about visas.

Yes, the exit/entries can be done at Ranong and get the full 60 days. It's just a case of crossing a border which you obviously do there.
Not sure of your brother's age or agility but if he's not too good at getting up and down ladders and onto rickety long-tail boats, he might want to consider crossing via the Andaman Club rather than at the main pier. The Club is a few kilometers out of Ranong (before you get to the town) and the boat is large, comfortable and easy to get on and off. It goes to an island/casino in Burmese territory and there's an Imm office on the Thai side so he'll get his 60 days there as well.
